The Case Manager, RN position will support patient and family payer representatives, medical and allied health team personnel and the organization in achieving optimal, timely and durable patient outcomes, optimal customer satisfaction, optimal management of resources and costs during the stay, outcome progression, outcome attainment and discharge of each patient, compliance with criteria and regulatory requirements. Ensures that appropriate, adequate services of value to the patient are delivered without discrimination. Will serve as a patient and family advocate to ensure customer needs and concerns are communicated and addressed in a timely manner. Serves as a liaison between the hospital and patient, family, physician, payer, rehabilitation team and external resources. Contributes to assessment and planning by determining the demands of the discharge setting upon the patient, and the total resources available to patient. Collaborates with the patient, family, payer, rehab team and community resources to ensure a successful transition to the discharge setting and durability of outcome. Reviews the medical record for complete and adequate documentation and compliance with admission and continued stay criteria. Works with payer sources to assure information is timely and accurate, verifies or obtains authorization for services.
Associates Degree in Nursing from an accredited institution required; Bachelor or Master’s Degree in Nursing preferred.
Minimum of 2 (two) years clinical experience in an acute care hospital environment, surgical center, or rehabilitation setting.
Prefer experience as a Case Manager or discharge planner.
